Terms of Service & Financial Policy
By receiving services from Divine Healing Healthcare, PLLC, you acknowledge that you have read, understand, and agree to the following Terms of Service and Financial Policy.
Patients are responsible for all applicable deductibles, copayments, coinsurance, non-covered services, and balances not paid by insurance.
Payment is due at the time services are rendered unless prior arrangements have been made.
A valid credit or debit card may be maintained on file and charged for authorized balances, membership fees, missed appointment fees, returned payments, and other amounts owed in accordance with signed patient authorizations.
As a courtesy, we submit claims to participating insurance companies. Verification of benefits does not guarantee payment. Patients remain financially responsible for any balance not covered by insurance.
Patients enrolled in a membership program agree to the Membership Agreement.
Membership fees are automatically billed to the payment method on file.
Membership fees are non-refundable once processed.
Membership benefits remain active only while the membership is in good standing.
Membership cancellation requests must be submitted according to the Membership Agreement before the next billing cycle.
Membership pricing and benefits are subject to change with reasonable notice.
Vitamin injections, wellness injections, IV therapies, and elective wellness services are self-pay unless otherwise stated.
Payment is due before services are rendered.
Packages are non-transferable and non-refundable.
Services may not be exchanged for cash.
Medical clearance may be required before treatment.
Divine Healing Healthcare reserves the right to refuse treatment if medically inappropriate or unsafe.
Medical weight loss medications, including compounded medications, are prescribed only when medically appropriate.
Insurance approval is not guaranteed.
Medication pricing may change without notice.
Compounded medications are custom prepared and cannot be returned or refunded once ordered, except where required by law.
Shipping and processing times vary depending on pharmacy requirements.
Appointments must be canceled at least 24 hours in advance.
Late cancellations and missed appointments may result in the clinic's current cancellation fee.
Repeated no-shows or excessive cancellations may result in dismissal from the practice.
Except where prohibited by law:
Membership fees are non-refundable.
Administrative fees are non-refundable.
Vitamin injections and wellness services are non-refundable after purchase.
Prepaid packages are non-refundable and non-transferable.
Prescription medications and compounded medications cannot be returned or refunded.
Shipping and processing fees are non-refundable after an order has been placed.
Use of our website, patient portal, Telehealth services, and online scheduling constitutes acceptance of these Terms of Service.
Patients are responsible for maintaining accurate contact information and protecting the confidentiality of their login credentials.
Divine Healing Healthcare, PLLC reserves the right to modify these Terms of Service and Financial Policy at any time. The most current version will be posted on our website and becomes effective upon publication.
